A fixed full-arch rehabilitation after 12 years of partial dentures

Maria wore an upper partial for 12 years. She had 4mm of bone on the left premolar region and a failing canine. We planned around what was left.

Treatment narrative

Maria arrived ready to hear 'not enough bone'. She had been told exactly that by two other clinics in the previous year.

Our CBCT showed 4mm of residual bone on the left and 9mm on the right. We planned six implants with a tilted distal on the left so the graft was unnecessary. Surgery ran 3h 40m under conscious sedation.

A screw-retained provisional was delivered on day 2. The final zirconia arch was fitted at month five after occlusal refinement. Maria kept her right canine as an over-denture support for the first three months to maintain proprioception.

  • No bone graft required
  • Same-day provisional arch
  • 5 implants, 1 tilted distal
  • Final arch delivered month 5
